The Cleveland Division of Police is highlighting a program designed to help families whose relatives are at risk of wandering or becoming lost due to cognitive conditions. The “Take Me Home” initiative offers a way for caregivers to register loved ones who may need assistance if they become disoriented or go missing.

Families can access the registration form online through the city’s official website (https://cleveland.gov/sites/clevelandohio/files/Public%20safety/Police/Take-Me-Home-Form-0624.pdf) and submit it by email to helpfindthemissing@clevelandohio.gov. This service aims to provide an extra layer of safety for vulnerable individuals in the community.

The announcement was made as part of Ride Along Radio, a biweekly program from the Cleveland Division of Police that gives listeners insight into ongoing police efforts and initiatives. The show airs every other Friday at 4 PM Eastern Time and is also available on YouTube (@ClevelandDivisionofPolice).

According to officials, “Take Me Home” has quietly impacted many lives in Cleveland by offering support and peace of mind to families concerned about their loved ones’ safety.
