                        March 31 -- 9 a.m. to 1 p.m.

                        (2)  Original Birth Certificate *

                        (3)  Immunization Records *

                        (4)  Parent\/Guardian ID *

                        (5)  $20 Processing Fee (cash or check) *

                        (6)  Proof of Residency: *

                                      -- Homeowners must bring a mortgage statement or deed

                                      -- Renters or lessees must bring an official, signed rental agreement

                                      -- Homeowners, renters and/or lessees must bring a current Utility Bill
