Pike-Delta-York Kindergarten Registration Process For the 2022-2023 School Year
Delta Elementary Kindergarten Screening will be April 28 & 29. During the screening, children will be assessed for kindergarten readiness in the areas of language, concepts, and
motor skills. Speech and language will be assessed by the speech pathologist. The nurse may do vision and hearing screenings also, if time permits. This entire screening will take
approximately 45 minutes.
If your child will be 5 years old before August 1, 2022, you may register online March 22nd through April 22nd at https://delta-oh.finalforms.com. Please have the following
documents ready before beginning the registration process, as they will need to be uploaded.
o Birth Certificate
o Custody Papers (if applicable)
o Immunization records
o Proof of residency for the Pike-Delta-York School District
(Samples of residency are current utility bills, driver’s license, payroll stub, home deed, or lease agreement)
Once FinalForms is completed and all of the required documents have been uploaded we will call you to schedule a screening appointment.
*The current kindergarten students will not have school April 28th & 29th.
*Screening appointments will be subject to availability for any registrations received after April 22nd.
