Where: Rootstown Elementary School
Who: Children who will be five (5) years old on or before August 1st
What to bring: You must have the following to enroll your child:
- Two (2) Proofs of residency (rental, lease or purchase agreement or mortgage statement, and utility bill). We cannot take a driver’s license, checks or credit cards.
- Birth Certificate
- Social Security Card
- Immunization records
- Custody papers, if applicable
