Dr. Ray Duffett and his staff will be conducting pre-participation physical exams for Canfield students at the High School on Wednesday, May 18, 2022, at 5:00 p.m. The cost is $7.00 per student (CASH ONLY).
You must bring the student's physical paper copy with you that day. The easiest way to obtain a paper copy is for you to print it from your Final Forms account. Log into your account, click on Forms (at top right), click on tab for OHSAA PPE Physical for Physicians (at bottom left), then click on the blue box (center page) saying "click to print OHSAA PPE Physical Form". The physical will already be filled out with the students info, and will just need the doctor to complete his portion.
At the completion of the physical, the doctor will give the physical form back to the student-athlete. It will then have to get uploaded onto their Final Form account by a parent.
